Irreversible pulpitis in upper 2nd Molar

If you're an Endodontist.... you can feel the pain in this xray photo
An every dentist nightmare 
Irreversible pulpitis in upper 2nd Molar with deep distal caries in limited mouth opening patient with working length 23-24 mm..
The mouth opening was too limited that i couldn't take a master cone xray...
I totally depend on my Sinirgia Doctor Scope Egypt Dr's Finder apex locator 
Some tips for dealing with limited mouth opening patient:
1- use rubber dam... on the contrary...it makes space and retract tongue and cheek 
2- use size 3 mirror ( the usual mirror size we use is 5)
3- use short files ( total length 21 mm) if possible 
4- use flexible files 
using Videya Dental TC Blue files with exceptional flexibility helps me a lot in such cases
