Apical Surgery could be a very good treatment choic in many cases.... as this one
A referred case of upper 1st premolar with broken file in the apical third of buccal root and under preparation and filling of palatal root..
Patient complaint was pain on bitting
Dealing with the broken file has 3 options:
1- Retrieval
The root was curved... that means sacrificing a lot of dentine to have a straight line view to the broken fragment taking a lot of time and effort with little chance ( for my humble experience in Retrieval) to success in Retrieval
2- Bypass
Due to severity of the curvature and the resistance I found in the gp filling the canal... bypass was unachievable and I started to feel the creation of a little ledge
3- Surgery
That was my final choice ...After cleaning and shaping the accessible part of the canal and obturation with MTA to save time and effort during Surgery........just open-cut-close
