A RCT of upper 1st premolar with acute irreversible pulpitis 
After working length radiograph I noticed that the buccal canal is not centered in the buccal root....
It seems very obvious that either we have extra buccal canal( disto-buccal) or extra buccal root ( molarized premolar)
I finished shaping the buccal and palatal canals 
And for 30 minutes.... I tried to find an orifice with ultrasonics or with scouting the buccal canal with Curved 10 k file.... with no results
So I stopped and asked for CBCT
It was a surprise.... the CBCT confirms the presence of only 2 canal... and what I thought is an extra canal or root is only an unusual shape of the buccal root...
